This volunteer agreement (“Agreement”) is entered into as of the date set forth below by Dispatch Dogs/Dispatch Trucking/Sahler Properties which from now on will be only referred to as Dispatch Dogs . WHEREAS, the Volunteer has voluntarily contacted Dispatch Dogs and has expressed an interest in working with dogs in the care and custody of Dispatch Dogs. In consideration of Dispatch Dog’s agreement to allow the volunteer to view and/or interact with such dogs, the volunteer hereby, for his/her heirs, his/her personal representatives, and him/herself represent and warrant as follows: 1. The volunteer is fully aware of the risks that dogs pose and have elected to view and/or interact with one or more dogs in the care of Dispatch Dogs voluntarily. The volunteer knowingly assumes all risks that exposure to dogs may pose, including but not limited to serious bodily injury and/or death. 2. THE VOLUNTEER HEREBY WAIVES, RELEASES DISCHARGES, HOLD HARMLESS, AND PROMISES TO INDEMNIFY AND NOT TO BRING SUIT AGAINST DISPATCH DOGS, its directors, officers, volunteers, and attorney for any of the referenced parties and any other parties acting for or on behalf of the referenced partied from an and all rights and claims which the volunteer has or which may hereafter accrue to the volunteer and from any and all damages which may be sustained by the volunteer directly or indirectly in connection with, or arising out of, the volunteer’s exposure to such dogs.

I want to volunteer my time and services for Dispatch Trucking/Dispatch Dogs/Sahler Properties hereby acknowledge that said organization is doing everything they can to protect the public as well myself as a volunteer. To this extent, I agree to follow Center of Disease Control (CDC) and local health district guidelines and Dispatch Dogs/Dispatch Trucking/Sahler Properties policies and procedures for social distancing to reduce the spread of Novel Coronavirus, or COVID-19. This will require me to maintain six (6) feet of distance between myself, fellow volunteers, and patrons of the organization as much as possible. This procedure will be required for visitor-to-visitor contact as well to limit exposure.
I agree to utilize surgical masks or improvised masks such as scarves, bandanas, and handkerchiefs to reduce the risk of exposure to myself and others. I agree to wash or sanitize my hands after using the restroom, sneezing and coughing.
I understand that I may be informed of or encounter sensitive Personal Health Information (PHI) for those that Dispatch Dogs/Dispatch Trucking/Sahler Properties serves. I agree to hold this information in confidence and will not disseminate any PHI except as allowed by law and/or per the policy and procedures of said organization which I am volunteering for.
I understand that there is no medical health coverage afforded to me during my relationship with Dispatch Dogs/Dispatch Trucking/Sahler Properties. Dispatch Dogs/Dispatch Trucking/Sahler Properties is not responsible for any potential exposure to Novel Coronavirus, or COVID-19, which is not a direct result of negligence on the part of their employees, volunteers, or the organization. Unless specifically stated in writing, I understand that there is no Minnesota State Labor and Industries employment security insurance provided to me.
By signing below, I agree to comply with the written instructions above. Failure to comply with these written instructions or verbal instructions from staff may result in my volunteer privileges being removed and I may be asked to leave the premises.
